According to art. 13 it. 1 and 2 of Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (general data protection regulation) ) (further as: "GDPR") we inform you that:
- The controller of personal data left in connection with the contact request is Medical University of Lodz, al. Kościuszki 4, 90-419 Łódź.
- Contact with the Data Protection Supervisor of the University is possible via the following e-mail address: iod@umed.lodz.pl.
- Your personal data are processed in order to answer your contact request , i.e. based on art. 6 paragraph 1 lit. b of GDPR.
- We do not disclose to anyone the data you provided in the contact form.
- Based on personal data, decisions will not be made in an automated manner, nor will they be subject to profiling.
- Your personal data will be stored for a period of 6 months from the date of sending a contact request.
- You have the right to lodge a complaint to the President of the Office for Personal Data Protection for unlawful processing of personal data by the Medical University of Lodz, the right to file a complaint only relates to the lawfulness of personal data processing.
- Providing your personal data is voluntary but necessary to answer your contact request.
- You have the right to access your personal data and the right to rectify, delete, limit processing, the right to transfer data, the right to object, the right to withdraw your consent at any time without affecting the legality of processing, which was made on the basis of consent before its withdrawal - within the limits set by law
